Custom Forms
Have questions for your families and not sure how to ask? We have the perfect feature for you! Popsicle's Custom Forms feature allows you to collect data at registration in a simple, seamless way--and the options are endless!
To access the Custom Forms feature, simply log into your Popsicle account and navigate to the Settings. Under the "Lists" section, you will see the Custom Forms option:
Click on "Create Form" to get started!
This is where you will choose a name for the Custom Form you are creating:
You can create questions based on the needs of your organization, and you will have the option to require an answer at registration (if applicable) here:
When creating Custom Forms questions, there are a number of formatting options available:
- Short Answer
- Long Answer
- Multiple Choice
- Checkboxes
- Drop Down Menu
Once you've added all of the questions you'd like to ask, click on "Save Form."
Once you have your Custom Forms in place in your Settings, they will be ready to use when creating your next activity! 🎉
You will see the option for additional details on the last screen while creating activities:
Once this option is toggled to the "on" position, you will be able to choose the applicable form that you would like families to see upon registration for that specific activity:
Upon registering, families will be prompted to answer the questions you have added/designated for this activity:
**Please Note: Our Custom Forms feature is currently in beta, meaning there may occasionally be some inaccuracies. We are continually working to improve the quality and accuracy of this feature over time, and appreciate your patience while testing.
